OT Assessment

We provide comprehensive occupational therapy assessments to better understand each individual's functional strengths, limitations, and support needs. These assessments guide therapy planning, funding applications, and care decisions.

Assessments include:

  • Reviews for ongoing care needs and progress

Intervention

We deliver evidence-based therapy to support individuals in regaining independence, improving participation, and achieving meaningful goals. Our approach is personalised, goal-directed, and responsive to each client’s unique presentation.

Focus areas include:

  • Upper limb rehabilitation for neurological conditions

  • Cognitive therapy (e.g. memory, executive functioning, problem-solving) and resource development

  • Fatigue management and daily routine structuring

  • Functional skill development for home and community life

  • Support for community access and engagement

  • Additional areas based on individual clinical needs

Reports & Documentation

We provide clear, evidence-based reports to support access to services, justify funding requests, and communicate therapy progress.

We complete:

  • NDIS functional capacity assessments and goal-based reports

  • Justification reports for assistive technology and home modifications (NDIS & TAC)

  • Summary and support needs reports for TAC - review of capabilities, community access, assistive technology

  • Progress updates and therapy reviews for ongoing care

  • Reports tailored to client, support coordinator, or funding body requirements

  • Additional services based on individual clinical needs

Assistive Technology & Home Modifications

We assess, trial, prescribe, and justify assistive technology and environmental modifications to improve safety, function, and independence in daily life.

Services include:

  • Equipment trials and prescription (mobility aids, daily living supports, cognitive aids)

  • Home modifications

  • Collaboration with builders, suppliers, and support coordinators

  • Comprehensive documentation to support funding applications

  • Training for clients, carers, and support teams

  • Additional services based on individual clinical needs
